List of documents in accordance with Article 30.5 of the Executive Authority Law
In accordance with Article 30.5 of the Law of the Republic of Azerbaijan “On Administrative Proceedings”, a list of documents or information that must be attached to an application, which can be obtained via the Electronic Government Information System by administrative bodies.
Appointment of a guardian for a natural person
Documents required for the service
- Documents confirming that the individual is incapable, due to their state of health, of independently exercising and defending their rights, and of fulfilling their duties.
- A copy of the identity card of the individual wishing to be placed under a guardianship.
- A notarised consent form from the proposed patron
- A copy of the identity card of the proposed patron
- A notarised consent form from the guardian the individual wishes to appoint
- Application from the individual wishing to be placed under guardianship
Registration of citizens in areas with a need for housing
Documents required for the service
- A notarised copy of the certificate of entitlement
- A notarised copy of the birth certificate
- A notarised copy of the marriage certificate
- A certificate confirming whether the citizen owns a flat or a plot of land
- A notarised copy of the identity cards
- Certificate-Form 2 (from the Executive Representation, MİS, or Co-operative Buildings)
- A certificate from the State Real Estate Registry (in the name of each of your family members over the age of 18)
- A certificate from the local council (regarding whether they own any land - for each of your family members over the age of 18)
- Copies of the identity cards of family members listed on the form 9.2/2 certificate
- If any family member is employed, a certificate from their employer stating whether they are on a housing waiting list
- Copies of the children's birth certificates
- Tenancy registration document for the flat or house
- Copy of the Medical-Social Expert Commission certificate of disability
- Copy of the Disability ID card or Martyr's Family ID card
- Tenancy agreement for the flat
Appointment of a guardian for minors aged from fourteen to eighteen, as well as for individuals whose capacity to act has been restricted by a court due to abuse of alcohol, drugs or psychotropic substances, or to involvement in gambling.
Documents required for the service
- Medical certificates for the guardian from the neuropsychiatric, oncology, narcology, tuberculosis, and dermatovenerology dispensaries
- Form No. 2 certificate of residence for the guardian
- Consent form from the minor
- A copy of the birth certificate of the adoptee's parents
- A notarised application from the guardian to the head of the Executive Authority
- The guardian's curriculum vitae
- Notarised consent forms from the adoptee's close relatives
- A copy of the guardian's marriage certificate
- A certificate of the ward's education
- A work reference and character reference for the guardian (if employed)
- A copy of the court's judgment on the limitation of a person's capacity to act, where that person has placed their family in severe financial hardship through the abuse of alcohol, drugs or psychotropic substances, or through gambling.
- A copy of the ward's birth certificate
- A copy of the ward's identity card
- Information about the parents (a copy of the death certificate, a copy of the court ruling on deprivation of parental rights, etc.)
Giving an Opinion (Permission) on the Property of Persons in Guardianship and Ward of the Court
Documents required for the service
- A copy of the guardian's or ward's identity card
- A copy of the guardian's or ward's birth certificate
- A copy of the documents relating to the ward's property
- A copy of the document confirming the appointment of the guardian
- A copy of the guardian's identity card

Granting consent for notarial acts to be carried out on movable and immovable property owned by a minor
Documents required for the service
- A copy of the minor's birth certificate or identity card
- A copy of the property documents
- A certificate of residence and family status
- A copy of the parents' identity cards

Granting permission to change a child's first name and surname
Documents required for the service
- Proof of residence (including the child's name)
- A copy of the death certificate for any deceased parent
- A copy of the child's birth certificate
- A consent form for the child, if they are over 10 years of age
- A copy of the parents' identity cards
- Copies of the applicants' identity documents
- A copy of the parents' marriage certificate (if applicable), or a copy of the divorce certificate, or a copy of the death certificate for a deceased parent
- A notarised joint application from the parents
Appointment of a guardian for minors under the age of fourteen, as well as for individuals adjudicated mentally incapable by a court
Documents required for the service
- A copy of the child's birth certificate
- Notarised consent forms from the adoptee's close relatives
- The guardian's curriculum vitae
- A court ruling on the incapacity of individuals who, due to a mental impairment or mental illness, do not understand the meaning of their actions or are unable to govern them
- A reference from the guardian's place of employment (with salary stated)
- Character reference from the guardian's place of employment (if employed)
- Certificate of the ward's education
- Form No. 2 certificate of the guardian's place of residence
- Information about the parents (copy of death certificate, copy of the court ruling on deprivation of parental rights, etc.)
- A notarised application from the guardian to the head of the Executive Authority
- A consent form from the child, if over the age of 10
- Medical certificates for the guardian from neurological, oncology, narcology, tuberculosis, and dermatological-venereal dispensaries
- A copy of the guardian's marriage certificate
- A copy of the adoptive parent's identity document
Documents required by the Service to retain the registration for the family members of a person with a disability who has been registered as in need of accommodation, in the event of their death.
